Loveland, OH - December 8,2025 — CommonSpirit Health at Home announced today the expansion of its home-based care services in the Greater Houston area with the addition of Post-Health Home Care Hospice.
This marks a significant milestone in the organization’s ongoing commitment to delivering accessible, high-quality care to patients and families across one of the nation’s largest and fastest-growing markets.
In October 2023, CommonSpirit Health at Home expanded its home-based health care services into the Greater Houston area through the acquisition of Post-Health Home Care, allowing Health at Home to offer skilled Home Health care.
This strategic step enabled the organization to support St. Luke’s Health’s patient population upon discharge across the region, extending our reach to communities in and around the Texas Medical Center, Sugar Land, The Woodlands, Pasadena, Pearland, Springwoods Village, Holcombe, Northwest Houston, and the Galleria area..
Over the past two years, CommonSpirit Health at Home has cultivated strong relationships with community partners and local health systems, laying the foundation for continued growth and expanded access to care. The addition of hospice services represents the next step in broadening the organization’s impact and serving more people in need throughout the region.
Heather Joe, FNP-C, Market Director of Operations for the Houston market, will oversee the new hospice program. In partnership with leaders from across CommonSpirit Health at Home, Joe will guide the development of a comprehensive, compassionate hospice service line tailored to the needs of patients and families facing end-of-life care.
The growing need for hospice care is driven by an aging population and hospice services allow patients and their families the ability to receive the care and comfort they desire, where they prefer to be, which is in their own home.
