CommonSpirit Health at Home Expanding to Offer Home Health Care Services in Houston, Texas

Loveland, OH - October 10, 2023 – CommonSpirit Health at Home, the national home health service line for CommonSpirit Health, has acquired Post-Health Home Care in Houston, TX to expand its home-based health care services in the Greater Houston area and strengthen its national footprint to care for more people in need.

The acquisition of Post-Health Home Care will enable CommonSpirit Health at Home to serve CHI St. Luke’s Health throughout the greater Houston market, including communities in and around the Texas Medical Center, Sugar Land, The Woodlands, Pasadena, Pearland, Springwoods Village, Holcombe, Northwest Houston, and the Galleria area.  

Continued health care industry changes require organizations to collaborate and construct relevant strategic imperatives that will advance the value proposition of home-based services. CommonSpirit Health at Home is committed to evolving to the needs of the industry and its patients while delivering specialized care for patients, cost-effectively in their home, with improved outcomes through patient-centered care. 

Post-Health Home Care will continue to partner with local health care providers to enhance the continuum of care for patients leaving the hospital and other care facilities. Leveraging the resources of CommonSpirit Health at Home, Post-Health Home Care will provide high-quality and innovative service to pre and post-acute patients, improve patient outcomes, increase patient satisfaction, reduce unnecessary admissions and readmissions, and support CommonSpirit Health’s efforts at reducing unnecessary costs and avoidable readmissions.

This addition supports CommonSpirit Health at Home’s strategy of growing into new communities, making Texas the 14th state for the company, expanding services in our current markets and creating new partnerships with health systems across our footprint to improve the health of patients and the community as a whole. 

Post-Health Home Care will assist CHI St. Luke’s Health and other systems in the market along with aligned physician groups and other partners who have a need for home-health services, while continuing to support the ongoing development of value-based systems of care in the community.
