Whether a patient is transitioning home from a hospital or nursing facility stay or is managing a chronic condition, our team ensures our patients are safe and they can retain their independence, in a familiar environment, during their recovery, while managing a chronic disease or dealing with multiple diagnoses.

Based on your doctor’s orders and your clinical needs, our team comes to your home to provide health care services such as skilled nursing, home health aides, physical, occupational and other therapies.

In partnership with your doctor, we develop a plan for you to manage medications, provide education to you and your family members about your condition and walk through the expected course of your illness.  We outline warning signs to watch for so we can intervene early and work with your doctor to treat potential problems.

Our nurses collaborate with each patient’s physician to provide one-on-one care and education to help patients improve their health faster and recover in the comfort of their own home.  We provide care based on our patients' individual treatment plan through the highest levels of quality, compassion and respect.

Our therapy services are designed to help patients who are recuperating at home from surgery or illness in order to improve flexibility, regain strength, and help maintain their independence. Our services also assist patients who may have cognitive or communication difficulties. Through education and comprehensive coordinated care, our therapy services address the unique needs of our patients as they rehabilitate wherever they call home.

Paraprofessionals are trained aides and attendants who assist in carrying out a patient’s plan of care that has been established by a professional clinical team member (such as a Registered Nurse or Physical Therapist) in coordination with the patient and their physician.  Our team will assist patients and their family members with information to identify additional resources that may be needed in the home while you recover.

Our Social Workers coordinate with physicians and caregivers to improve access to community resources in order to maintain and enhance the social, emotional, functional, and physical health status of our patients.

Registered Dieticians work with each patient’s physician to establish a plan of care to meet the specific needs of each patient. Our Dieticians collaborate with our entire home health team, providing education to patients and caregivers, along with quality services that promote nutritional health and effective dietary patterns for our patients.

Specialty services

CommonSpirit Health at Home offers a number of specialty services to help our patients safely transition care to their home and recuperate in their preferred setting.  Working directly with physicians, we are able to manage a variety of diseases and conditions in a home care setting through qualified and compassionate care.

Our Home Care speciality services are designed to support patients who are at a high risk of rehospitalization after discharge. Our Chronic Care Management Programs are designed by experts to provide tailored care for common, chronic diseases such as heart failure, pulmonary obstruction, diabetes, and more. Patients are assessed for this specialty program and receive tailored interventions and care plans.
